Employees were working from an unapproved and unsecured work platform constructed of stacked plywood while engaged in residential construction activities at heights of approximately 12 to 15 feet above grade.

Employees were working from an unapproved and unsecured work platform constructed of stacked plywood while engaged in residential construction activities at heights of approximately 12 to 15 feet above grade without any means of fall protection.  J&M Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard. 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 1291571, citation number 1, item number 2 and was affirmed as a final order on July 24, 2018, with respect to a workplace located at 690 Forest Drive, Teaneck, NJ 07666.
