Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck-by inadequately secured loads. On or about October 27, 2015, and at times prior thereto; a. Employees in Building B were exposed to struck-by hazards while operating a Dayton, 1 ton, underhung hoists without a functioning safety latch on the hoisting hook. b. Employees in Building B were exposed to struck-by hazards while operating a Crane Mann brand 3 ton, overhead crane without a functioning safety latch on the hoisting hook. c. Employees in the facility were exposed to struck-by hazards while operating overhead cranes with deficiencies, that had not been inspected periodically and frequently by the employer.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. On or about October 27, 2015, and at times prior thereto, employees in building A and B were exposed to struck-by, caught-in and electrical hazards when performing service or maintenance on horizontal lathes, vertical lathe, oven and decanter centrifuges without implementation of an energy control program. Alfa Laval, Inc. was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1910.147(c)(1), which was contained in OSHA inspection number 191158 Citation Number 1, Item Number 1 and was affirmed as a final order on 5/22/2012, with a final abatement date of May 14, 2012, with respect to a workplace located at 321 Foster Avenue, Wood Dale, IL 60191.
