OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and  a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazards:  On or about 10/21/2015, employees engaged in the task of cutting grass with a John Deer Quick Track Pro 652B Lawn Mower were exposed to amputation hazards in that the area where the employee was operating  the mower had not been surveyed for possible hazards that could cause employees to fall from the mower and make contact with the blade.  Among other methods, one feasible and acceptable method to correct this hazard is to provide employees with training on equipment controls, components, their functions, and limitations; and instruct employees on the safe operating procedures as outlined in the equipment operator's manual.
