29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) EMKA, Inc., 1961 Fulling Mill Road, Middletown, Pennsylvania - Employees did servicing and maintenance activities, such as tool and die changes, on machinery and equipment, including, but not limited to, the Normec Combo Press (vulcanizing machine), the Maschinenbau Kubitza saw (saw for cutting gaskets), and the 75-ton Minster 7 part revolution mechanical power press (rod 75 ton press).  The employer did not develop and implement machine specific energy control procedures for these machines, and employees did not lockout/tagout these machines during the tool and die changes, exposing employees to hazards, including but not limited to, getting caught-in and struck-by moving machine parts, on or about October 20, 2015.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.217(d)(9)(i): The employer did not establish a die setting procedure for mechanical power presses that would ensure compliance with 29 CFR 1910.217(c):  (a) Warehouse - Employees did die changes on the 75-ton Minster 7 part revolution mechanical power press (rod 75 ton press), and the employer did not develop and implement a die setting procedure for this machine, exposing employees to the hazards of getting caught-in and struck-by moving machine parts, on or about October 29, 2015.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.217(c)(3)(viii): Two-hand trip devices used on mechanical power presses did not protect the operator in that the devices were not located at such a safety distance from the point of operation that the slide completed its downward travel or stopped before the operator could reach into the point of operation with his hands:  (a) Warehouse - The 75-ton Minster 7, part revolution mechanical power press, serial number 7SS-28583, which operated at 90 strokes per minutes, had two-hand trips as the point of operation safeguarding device, with the left palm button located 14 inches rather than 21 inches from the point of operation, exposing employees to hazards of getting caught-in and struck-by moving machine parts in the point of operation, on or about October 29, 2015.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.217(c)(3)(viii)(d): Two-hand trip devices of mechanical power presses were not fixed in position so that only a supervisor or safety engineer was capable of relocating the controls:  (a) Warehouse - The 75-ton Minster 7, part revolution mechanical power press, serial number 7SS-28583, which operated at 90 strokes per minutes, had two-hand trips as the point of operation safeguarding device.  The employer did not have the two-hand trip device in a fixed position so that only a supervisor or safety engineer had the capability of relocating this control.  Employees moved the two-hand trip device to various locations depending on the die in use, exposing employees to hazards of getting caught-in and struck-by moving machine parts, on or about October 29, 2015.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.217(d)(5): Openings between the top of punch holders and the face of slides or striking pads on mechanical power presses were not safeguarded in accordance with the requirements of paragraph (c) of this section:  (a) Warehouse - An opening 5.5 inches high existed between the top of the unitized die and the striking pad on the 75-ton Minster 7, part revolution mechanical power press, serial number 7SS-28583.  This press had two-hand trips with the left palm button located 14 inches rather than 21 inches from the opening between the top of the unitized die and the striking pad, exposing employees to hazards of getting caught-in and struck-by moving machine parts, on or about October 29, 2015.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
