29 CFR 1910.255(b)(4): All press welding machine operations, where there is a possibility of the operator's fingers being under the point of operation, were not effectively guarded by the use of a device such as an electronic eye safety circuit, two hand controls or protection similar to that prescribed for punch press operation.      a) Spot Welding Area - On or about October 7, 2015, an employee sustained an amputation injury because the employer did not effectively guard the point of operation on the Sciaky (SCKY04/2) hardware inserter press welding machine.     b) Spot Welding Area - On or about October 23, 2015, an employee sustained an amputation injury because the employer did not effectively guard the point of operation on the Sciaky (SCKY01/5) press welding machine.       c) Spot Welding Area - On November 5, 2015, the employer did not effectively guard the point of operation on the Sciaky (SCKY04/2) hardware inserter press welding machine exposing employees to point of operation hazards.      In accordance with 29CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.
