29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about October 7, 2015, in the Molding Department, the employer allowed employees access to the unguarded latch-receiving housing unit of the 2015 Beckwood Press Horizontal Compression Machine (HCM) Serial #32909 during the operation of the machine. An employee placed in hand into the latch receiver housing and lost two fingers when the housing pin fired inside of the housing unit.
