29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a) Orbital ATK, Inc. dba New River Energetics: On or about 10/21/2015, an employee held emery cloth that was wrapped semi around an aluminum shaft fastened into the work holding device of an industrial lathe. When the shaft was rotated at 300-400 RPM, the emery cloth created an entanglement hazard that was not guarded as a result the employee received a fractured left hand and a severe laceration to the face. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.253(e)(3)(ii)(C)(3): Flashback protection was not provided by an approved device that would prevent flame from passing into the fuel gas system: a) Orbital ATK, Inc. dba New River Energetics, Army Radford Ammunition Plant Building 1513 Maintenance shop: On or about 09/29/2015, oxygen and acetylene gas cylinders attached with hoses, regulators and a Harris model no. 63-2 torch was used to cut an I-beam. The fuel gas was not provided with a flashback protective device, thus exposing employee to possible burns. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors were not replaced: a) Orbital ATK, Inc. dba New River Energetics, Army Radford Ammunition Plant Building 1513 Maintenance shop: On or about 09/29/2015, an electrode holder cable used in welding brackets was repaired with black tape in four different areas along the outer jacket of the cable and within 10 feet of the electrode holder to cover the exposed copper wires, thereby exposed an employee to a possible electrical shock. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
