29 CFR 1910.119(d): The employer did not complete a compilation of written process safety information, including information pertaining to the hazards of the highly hazardous chemicals used or produced by the process, information pertaining to the technology of the process, and information pertaining to the equipment in the process before conducting the process hazard analysis:     The employer is failing to protect the employees form the hazard of explosion by failing complete a compilation of process safety information for the covered processes.  This was most recently documented at 25201 East 78 Highway, Independence, Missouri when an employee was fatality injured while breaking up Tetrazene inside the vacuum receiver       The process uses a spatula and the company failed to compile information on the material of construction and how this information may impact the use of the spatula with tetrazene.

29 CFR 1910.119(e)(3): The process hazard analysis did not address the elements of (i) through (vii) of this paragraph:        The following are examples of hazards not addressed in the Process Hazard Analysis:       (a)   Manipulating and transferring a full cloth of dried (Tetrazene) within the muslin cloth from the vacuum receiver to the weighing table without knowing the moisture content of the Tetrazene in . Transferring Tetrazene from the vacuum receiver to the weighing table without knowing whether the Tetrazene is within safe handling limits exposes high explosive mixers to explosion hazards.      (b)  The wedging action (using a spatula to break up the tetrazene  cake in the dryer) utilized in the  Tetrazene de-watering process was not addressed in the Process Hazard Analysis.

29 CFR 1910.119(f)(1)(i): The employer did not develop and implement written operating procedures that provided clear instructions for safely conducting activities involved in each covered process consistent with the process safety information that addressed steps for each operating phase:      The employer is failing to protect employees from the hazards of explosion and fire by failing to include all manufacturing steps within the Tetrazene standard operating procedure.  This was most recently documented at 25201 E. 78 Hwy, Independence, Missouri when an employee was fatality injured while breaking up Tetrazene inside the vacuum receiver in .      The following are examples of missing steps within the Tetrazene standard operating procedure:       (a)  Wedging steps were not provided for the high explosive mixers who break up Tetrazene, an explosive compound that is dried inside the vacuum receiver in .      (b) Step by step instructions detailing how to safely transfer tetrazene the vacuum receiver to a conductive black plastic bag lining a rubber bucket or transferring the tetrazene from the black plastic bags to the vacuum receiver in were not provided.
