29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.    a). Office Building, 2nd Floor - On or about and times prior to October 29, 2015, a fall protection system was not provided to employees working near window openings, thus exposing employees to a fall hazard of approximately 11.1 feet onto compacted earthen ground.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  a). Office Building - On or about October 29, 2015, employees were installing metal decking on the roof without a fall protection system in place, thus exposing employees to a fall hazard of approximately 25 feet onto compacted earthen ground.

Note: If such modifications or changes were made, the capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly.    a) Office Building - On or about October 29, 2015, the employer did not obtain written manufacturer's approval for the use of a personnel work platform attachment that was utilized with a JLG Material Handler (model: G6-42A), thus exposing employees to a fall hazard of approximately 25 feet onto compacted earthen ground.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge.    a). Office Building - On or about and times prior to October 29, 2015, the stairways used to gain access to the second floor were not equipped with a stairrail system along the unprotected sides of the stairways, thus exposing employees to trip and fall hazards.
