29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): A) Middle area of shop, on or about June 5, 2020, and/or times prior to, the point of operation of the Bradbury 13 foot folding machine, was not adequately guarded in that the operator could cause the top plate to come down without resetting the machine and or clearing the laser sensor, after the laser sensor stopped the cycle, thereby exposing the employee to an amputation hazard.
