29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  a) James Coe Wood Products Co., Inc. - Gibsonburg, Ohio: On or about October 12, 2015, the employer failed to ensure that employees who accessed the top of the silo were protected from falling to the ground below. An employee was exposed to a fall of approximately 14.75 feet when accessing the top of the silo without a personal fall arrest system or guardrail in place.

29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    a) James Coe Wood Products Co., Inc. - Gibsonburg, Ohio: On or about November 5, 2015, the employer failed to implement the elements of a hearing conservation program for those employees overexposed to occupational noise. An employee working as a laborer on the saw mill was exposed to occupational noise at an 8-hour time weighted average of 97.7 dBA, which exceeded the OSHA action level of 85 dBA and the OSHA permissible exposure limit of 90 dBA.     b) James Coe Wood Products Co., Inc. - Gibsonburg, Ohio: On or about November 5, 2015, the employer failed to implement the elements of a hearing conservation program for those employees overexposed to occupational noise. An employee working as a pallet machine operator was exposed to noise at an 8-hour time weighted average of 91.5 dBA, which exceeded the OSHA action level of 85 dBA and the OSHA permissible exposure limit of 90 dBA.    c) James Coe Wood Products Co., Inc. - Gibsonburg, Ohio: On or about November 5, 2015, the employer failed to implement the elements of a hearing conservation program for those employees overexposed to occupational noise. An employee working as a pallet machine operator was exposed to noise at an 8-hour time weighted average of 91.4 dBA, which exceeded the OSHA action level of 85 dBA and the OSHA permissible exposure limit of 90 dBA.
