Route 20, Gibsonburg, OH; On or about November 2, 2015, the employer did not assure an energy control program, consisting of energy control procedures, training and periodic inspections was provided to employees. Employees are required to perform setup and maintenance activities on the pallet machine and    the descrambler and are exposed to hazards from unexpected start up.

Employees are required to work around the cut off saw and are exposed to amputation hazards in that the side of the guard is missing.

Route 20, Gibsonburg, OH; On or about November 2, 2015, the employer did not assure employees were protected while walking over the pallet stacker. Employees are required to use a walkway with wooden stairs in which they walk over the pallet stacker, exposing them to caught by hazards from the chain of the pallet stacker.            b.) James Coe Wood Products Company, Inc. located at 4694 US. Route 20, Gibsonburg, OH; On or about November 2, 2015, the employer did not assure employees were protected while working near the chain conveyor on the Descrambler. On October 19, 2015, an employee reached into the chain to adjust a nut for the guide piece and was caught between the chain and sprocket, resulting in an amputation of the left index finger.      c.) James Coe Wood Products Company, Inc. located at 4694 US. Route 20, Gibsonburg, OH; On or about November 2, 2015, the employer did not assure employees were protected while working around the chain conveyor at the end of the Saw Mill Line. Employees are required to stack cut wood pieces after moving through the cut saw and the gang saw, and are exposed to caught by hazards from the chain conveyor.
