29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) Great Lakes Excavating, Inc. / 8158 Bargain Road, McKean, PA 16426: On or about November 2, 2015 an employee was in a trench that was approximately 57 inches deep and 216 inches long without immediate access to a ladder or other means of egress.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. a) Great Lakes Excavating, Inc. / 8158 Bargain Road, McKean, PA 16426: On or about November 2, 2015 employees were working in a trench in which the spoil pile was not two feet or more away from the edge of the excavation. The trench was approximately 18 feet in length with the spoil pile immediately adjacent to the length of the trench. Approximately half of the spoil pile was approximately 1 foot from the edge of the excavation.
