29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: A. Great Lakes Excavating Inc. worksite located at 7220 Dutch Rd., Waterville, Ohio: On or about February 2, 2017, the employer did not ensure an employee was adequately protected from cave-ins while installing a waterline in a trench approximately 5.20 feet deep. The employee was working outside of the protection of the trench shield.
