29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: (a) For employees operating inside the Anodizing Bay at 369 Douglas Rd E, Oldsmar, FL 34677, as noted on or about 11/17/2015, employees were offered the 3M, N95, Sanding and Fiberglass Respirator, without having been provided the Appendix D information.
