29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: (a) On or about November 2, 2015, and at times prior, the electrical outlet supplying power to the EZLift Floor to Floor convey belt, was missing its faceplate. The outlet is energized and is next to the conveyor belt's up/down/stop control box where workers regularly reach. Employees are exposed to shock and electrocution hazards. (b) On or about November 2, 2015, and at times prior, the electrical junction box at the top of the EZLift Floor to Floor conveyor belt that is mounted next to the take-up roller at the top of the conveyor, is missing its face plate exposing workers to shock and electrocution hazards.
