(a)  225 Cross Farm Lane, York Pennsylvania - An employees perform maintenance on a conveyor chain that was not locked out, receiving an amputation to the finger tip.   The employer did not provide adequate training to ensure that employees could recognize hazardous energy and take the appropriate steps to isolate and control the energy, on or about October 28, 2015.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
