29 CFR 1910.23(e)(7)(iii): Covers projecting more than 1 inch above the floor level were used when all edges were not chamfered to an angle with the horizontal of under 30 degrees and all hinges, handles, bolts, or other parts were not set flush with the floor or cover surface: a) On or about November 4, 2015 the Wash Bay pit covers were not chamfered along the edges to prevent a tripping hazard, when the covers bowed up higher than 1 inch from the concrete floor. The covers were not secured to prevent accidental displacement and fell into the pit when not positioned properly.
