29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:   On or about November 2, 2015, during the installation of roofing material at 4913 Snow Rd, Parma, Ohio 44134; the employer did not conduct frequent and regular inspections of the job site where employees were exposed to fall hazards from working surfaces with a height of approximately 10.6 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about November 2, 2015, during the installation of roofing material at a job site located at 4913 Snow Rd, Parma, Ohio 44134; the employer did not ensure portable ladders extend 3 feet above the upper landing surface.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about November 2, 2015, during the installation of roofing material at a job site located at 4913 Snow Rd, Parma, Ohio 44134; the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 10.6 feet by utilizing a fall protection system.   The Custom Roofing of NE Ohio was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13)), which was contained in OSHA inspection number 950425, citation number 1, item number 1 and was affirmed as a final order on (7/9/2014), with respect to a workplace located at 4501 Cliffview Dr. Independence, Ohio 44131.
