29 CFR 1926.20(b)(1):The employer did not initiate and maintain accident prevention programs as necessary to comply with this part: On or about March 30, 2021, during roofing work, at a site located at 8666 Wiese Rd, Brecksville, OH 44141, the employer had not developed and implemented a safety and health program for employees to recognize and address unsafe working conditions and falls greater than 6 feet at the work site.

29 CFR 1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. On or about March 30, 2021, an employee was observed working on roof of a single family home at a job site located at 8666 Wiese Rd, Brecksville, OH 44141. The employer did not ensure exposed employees were protected from falls by utilizing a PFAS or protected by any other form of fall protection system at an approximate fall height of 10 ft.
