29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Bay 7-  (a) Employee(s) were observed using the Jet horizontal band saw, model HBS-1018W, sn# 07051550 while the saw blade's unused portion was unguarded, exposing operators to serious injuries.

29 CFR 1915.56(b)(2): Welding cable was not free from repair or splices for a minimum distance of 10 feet from the cable end to which the electrode holder was connected.  (a)  Building 72:  On or about Oct 28, 2015 and times prior thereto a welding cable was cut to the conductor 6 feet from the work lead.  This exposed employees to potential electrical shock hazard if employee contacted conductor during welding operations.  (b)  New Tugboat build:  On or about Oct 28, 2015 and times prior thereto a welding cable was cut to the conductor 8 feet from the work lead.  This exposed employees to potential electrical shock hazard if employee contacted conductor during welding operations.  (c)  New barge build:  On or about Oct 28, 2015 and times prior thereto a welding cable was cut to the conductor.  This exposed employees to potential electrical shock hazard if employee contacted conductor during welding operations.

29 CFR 1915.504(b)(3): The employer did not post a fire watch when there was combustible material less than 35 feet in either the horizontal or vertical direction that could not be removed, was not protected with flame-proof covers or shielded with metal fire-resistant guards or curtains.  Harley 83 barge tank 4:  On or about October 28, 2015 and times prior thereto, two employees were working in a tank with combustibles in the tank and there was no fire-watch or fire extinguisher in the tank.  This exposed the two employees to burns and smoke inhalation hazards.

29 CFR 1910.305(j)(2)(v): A receptacle installed outdoors in a location protected from the weather or in other damp locations did not have an enclosure for the receptacle that was weatherproof when the receptacle was covered (attachment plug cap not inserted and receptacle covers closed.  Weatherdeck of new barge build:  On or about October 28, 2015 and times prior thereto, weather covers were missing off of the electrical junction boxes.  This exposed employees to potential electrical hazards if water penetrated into the electrical circuitry.
