29 CFR 1910.136(a): The employer did not ensure that each affected employee used protective footwear when working in areas where there was a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, and where such employee's feet were exposed to electrical hazards: a) Cupola deck; On or about October 27, 2015 and at times prior thereto, the employer exposed employees to burns from molten rock in that employees working on the Cupola deck were not provided with adequate foot protection that would protect them from the heat, melt, and potential for burns identified in the company's job hazard assessment.

29 CFR 1910.1030(f)(1)(i): The employer did not make available the hepatitis B vaccine and vaccination series to all employees who have occupational exposure: a) Throughout the facility; On or about October 27, 2015 and at times prior thereto, the employer exposed his employees to potentially becoming infected with hepatitis b in that employees who had been trained as emergency responders and were expected to perform CPR/1st Aid if an accident occurred had not been offered the hepatitis B vaccination series as required by the regulation.
