29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. Employees performing residential roofing operations were exposed to struck by hazards while employees handed plywood sheets to the second level from the first level. The employer failed to mandate the use of hard hats. No abatement certification or documentation is required for this item.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows: Employees performing residential construction operations were exposed to a fall of 18.1' when the employer failed to ensure the scaffolding was fully planked. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part. Employees working from a scaffold on a residential building were exposed to a 18.1' fall hazard when the employer did not ensure guardrail systems were installed along all open sides and ends of platforms. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
