29 CFR 1926.59 referencing 29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a)  At the jobsite, accessing the roof, on or about 10/22/15:  Employees used liquid adhesives and aerosol adhesives where SDS's were not available at the jobsite, and where a written HAZCOM program was not established or implemented.    Abatement certification must be submitted for this item.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems:  a)  At the jobsite, on the 5th floor, on or about 10/22/15:  Employees transited this area multiple times per day to access and egress the roof area, while an unprotected hole was in close proximity to the walking path from the top of the stairs to the ladder accessing the roof.
