29 CFR 1926.652(c)(4)(i): Support systems, shield systems, and other protective systems not utilizing Option 1, Option 2 or Option 3, were not approved by a registered professional engineer: On or about 10/30/15 Location: IFO, 57 North Central Avenue, Hartsdale, NY 10530 a) Employees repairing a water main in a 10 foot wide, 17 foot long, 10 foot deep excavation were not protected by a protective system approved by a registered engineer. The excavation was in material, K-Crete Lean Mix, that does not fall into a OSHA soil or stable rock category allowing for the use of Options 1, 2 or 3. b) Employees repairing a water main in a 10 foot wide, 17 foot long, 10 foot deep excavation were not protected by a protective system approved by a registered engineer. The timber shoring provided did not meet the requirement utilizing Option 1, Appendix A, Soil type A (Least Protective) in that 3x10 pieces were used as cross braces instead of 6x6 pieces and the ends of the excavation where the pipe entered/exited was not protected. The use of a system lower in protection strength than the lowest rated OSHA system was not approved by a registered engineer.
