29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions: a) Job site, 10332 Perry Highway, Wexford, PA: On or about November 5, 2015, during water line repair activities, the employer did not ensure that an appropriate excavation inspection was conducted in that soil classification was not properly analyzed and no excavation measurements for depth, width, wall angles or other relevant dimensions were taken. The excavation was measured to be at least approximately 5 feet, 4 inches in depth and no appropriate cave in protection or egress system was in use while working in it.
