29 CFR 1926.652(d)(2): Manufactured materials and equipment used for protective systems were not used and maintained in a manner that was consistent with the recommendations of the manufacturer: (a) Job site, 5839 Pierce Street, Pittsburgh, PA: On or about June 30th 2021 employees were not protected from cave in hazards while using an Ultra Shore S Series trench box. The box was being used in an excavation that was approximately 16 feet in depth, while the tabulated data for this particular box states : In no case can the "S" series shield be used in excavations exceeding 12 feet in depth without conducting the proper earth pressure calculations.

29 CFR 1926.652(g)(1)(ii):Shields were not installed in a manner to restrict lateral or other hazardous movement of the shield in the event of the application of sudden lateral loads: (a) Job site, 5839 Pierce Street, Pittsburgh, PA: On or about June 30th 2021 employees were not protected from cave in hazards while using an Ultra Shore S Series trench box. The box was being used in an excavation that was approximately 16 feet in depth, the box was not supported in order to prevent any inadvertent lateral movement of the trench box.
