29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stair rail system along each unprotected side or edge.    (a) Coombs Farm Estates:  On or about November 4, 2015, and times prior, employees use the wood stairs to gain access to the 2nd story/top floor of a residential home, measuring 10ft. 2in. from the top of the stairs to the lower level with a total of 14 risers that was not equipped with a stair rail on the unprotected sides and/or edges.

29 CFR 1926.1052(c)(12): Unprotected sides and edges of stairway landings were not provided with guardrail systems that meet the criteria contained in Subpart M of 29 CFR 1926.  (a) Coombs Farm Estates: On or about November 4, 2015, and times prior, employees were working on the 2nd story of a residential house near a 7ft. 4in. by 8ft. 2in. unprotected stairway opening in the floor with an approximate distance of 18ft. 4in. from the top of 2nd story wall to the 1st story floor and 10ft. 2in. from the 2nd story landing and/or floor to the first story floor.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    (a) Coombs Farm Estates:  On or about November 4, 2015, and times prior, 3 employees were observed performing framing activities approximately 32 feet above the ground without the use of a fall protection system.     The K&F Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 315224469, citation number 2, item number 2 and was affirmed as a final order on August 18, 2011, with respect to a workplace located in Morgantown, WV.
