Arcing effects was not taken into consideration in determining the safety of the equipment.  (a) Located in the machine house of crane #10:  On or about and at times prior to switches, fuses and other exposed live electric parts were not adequately shielded to prevent serious injury to employees in the event of an arc flash incident.

(a) Located in the basin of dry dock #3: On or about and at times prior October 27, 2015 the inspection revealed that on a portable welding system outlet #8 was missing the weather cover.  (b) Located at the US Fab building way next to the barge under construction on the port side, midway:  On or about and at times prior October 27, 2015 the inspection revealed that on the load center #3 electrical box two of the receptacle outlet boxes were missing weather cover.  (c) Located on the port side wing wall of dry dock #3 at frame 95: On or about and at times prior to October 27, 2015 the inspection revealed a portable panel with damaged outlet.

29 CFR 1915.152(a): The employer did not provide or ensure each affected employee with the appropriate personal protective equipment (PPE) for the eyes, face, head, extremities, torso, and respiratory system, including protective clothing, protective barriers, personal fall protection equipment, and life saving equipment, meeting the applicable provisions of Subpart I, wherever employees were exposed to work activity hazards that required the use of PPE.    (a) Located at berth 312 at the waterside edge of the pier:  On October 26, 2015 an employee were potentially exposed to serious injury or death because the employer did not ensure that the employee wore his life jacket when exposed to a fall into the water of about 30 feet.   (b) Located at berth 313 at the waterside edge of the pier: On October 26, 2015 four rigging employees were potentially exposed to serious injury or death because the employer did not ensure that the employees wore their life jackets when exposed to a fall into the water of about 30 feet.

29 CFR 1910.1025(d)(6)(iii) is applicable to shipyard employment by 29 CFR 1915.1025 that incorporates the General Industry requirement: Where the initial monitoring revealed that employee exposure to lead was above the permissible exposure limit, ; monitoring was not repeated at least quarterly and continued at the required frequency until at least two consecutive measurements taken at least 7 days apart were below the PEL and at or above the action level:  (a) Located at Vigor Shipyard in Portland Oregon: On October 28, 2015 the inspection revealed that the employer did not conduct industrial hygiene monitoring to determine employee exposure levels to lead at least quarterly after sampling conducted by PBS Environmental on October 24, 2014 revealed an employee exposure to lead greater than OSHA's Permissible Exposure Limit (PEL).
