29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a)  Jobsite:  On or about November 4, 2015, one employee was not tied off and a second employee was tied off using a rock climbing seat harness while they were installing asphalt shingles on a new residential house.  the fall  hazard to the two employees was about 20 feet to the ground.  The pitch of the roof was about 7 in 12.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employees knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill:   (a)  Jobsite:  On or about November 4, 2015, one employee was doing roofing roof on a residential house that was under construction and he was not tied off.  A second employee, who was also doing roofing work on the same house was wearing a rock climbing seat type harness for fall protection.  The fall hazard to the two employees was about 20 feet to the ground.  These two employees need to be retrained so that they retain the necessary skills and knowledge to minimize fall hazards while working on roofs.
