29 CFR 1910.1018(c): Employee(s) were exposed to inorganic arsenic at concentrations greater than ten micrograms per cubic meter of air, averaged over an 8-hour period: (Construction Reference: 1926.1118)    Jobsite:   On 11-13-15 A worker was exposed to 26 ug/m3 of inorganic arsenic in excess of the 8 hour time weighted average permissible exposure limit(PEL) of 10ug/m3 during bridge abrasive blasting operations.     On 11-13-15  A worker was exposed to 29 ug/m3 of inorganic arsenic in excess of age 8 hour time weighted average permissible exposure limit (PEL) of 10 ug/m3during bridge abrasive blasting operations.     On 11-13-15  A worker was exposed to 12 ug/m3 of inorganic arsenic in excess of the 8 hour time weighted average permissible exposure limit(PEL) of 10 ug/m3 during bridge abrasive blasting operations.    On 11-13-15  A worker was exposed to 18 ug/m3 of inorganic arsenic  in excess of the  8 hour time weighted average permissible exposure limit of 10 ug/m3.      On 11-13-15  A worker was exposed to 16 ug/m3 of inorganic arsenic in excess of the 8 hour time weighted average permissible exposure limit of 10 ug/m3.

29 CFR 1926.62(c)(1): The employer did not ensure that no employees were exposed to lead at concentrations greater than fifty micrograms per cubic meter of air (50 ug/m3) averaged over an 8-hour period:    Jobsite: On 11-13-14  A worker was exposed to an 8 hour time weighed average level of lead exposure of 17,400 ug/m3 in excess of the of 50ug/m3 permissible exposure limit during abrasive blasting lead paint removal.  Sampling was performed for 443 minutes with zero exposure assumed for time not sampled for a total 8 hours.     The worker's total  exposure was 67,400 ug/m3 without regards to respiratory protection. Workers respiratory protection worn provided protection up to 50,000 ug/m3.

On 11-13-14  A worker was exposed to an 8 hour time weighed average level of lead exposure of 17,400 ug/m3 in excess of the of 50ug/m3 permissible exposure limit during abrasive blasting lead paint removal. 29 CFR 1926.62(i)(3)(ii): Where shower facilities were available, the employer did not ensure that employees showered at the end of the work shift:    Jobsite: Workers were not required to shower after the work shift where a shower trailer and change area was available for use. Employer transported workers to the hotel site with out ensuring showering. Workers were exposed airborne lead in excess of the permissible exposure limit of 50 ug/m3 during abrasive blasting lead paint removal.

29 CFR 1926.62(i)(4)(iv): The employer did not ensure that employees did not enter lunchroom facilities or eating areas with protective work clothing or equipment unless surface lead dust had been removed by vacuuming, downdraft booth, or other cleaning method that limited the dispersion of lead dust:    Jobsite:  Employer did not ensure that vacuum facilities were available and used by workers prior to food breaks, where airborne lead exposures exceeded the permissible exposure limit of 50 ug/m3.

29 CFR 1926.62(d)(8)(ii): Whenever the results indicate that the representative employee exposure, without regard to respirators, is at or above the PEL the employer shall include in the written notice a statement that the employees exposure was at or above that level and a description of the corrective action taken or to be taken to reduce exposure to below that level.    Jobsite: Employer did not ensure that workers exposed to lead were provided air sampling results and that workers exposed over the lead PEL were given a written notice concerning airborne exposure results and correction action being taken to reduce such workplace exposures during abrasive blasting operations.
