29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) On or about 11/09/15, on a residential construction jobsite at 650 Columbus Parkway, Buffalo, NY: Ladder jack scaffolding was erected on the south side of the house. Employees were on the "piks" of the scaffolding, transporting sheets of plywood to employees on the roof. The employees were exposed to a fall of approx. 20 ft. to the ground. No personal fall arrest system or other fall protection system was utilized. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 11/09/15, on a residential construction jobsite at 650 Columbus Parkway, Buffalo, NY: Ladder jack scaffolding was erected on the south side of the house. Employees used extension ladders to access the "piks" on the scaffolding. The employees were exposed to a fall of approx. 20 ft. to the ground because the siderails of an extension ladder used to access the west side of the scaffolding only extended approx. 18 inches above the edge of the pik. and the siderails of an extension ladder used to access the east side of the scaffold only extended approx. 30 inches above the edge of the pik". NO ABATEMENT CERTIFICATION REQUIRED

No personal fall arrest system or other fall protection system was utilized. NO ABATEMENT CERTIFICATION REQUIRED
