29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) On or about 03/26/18, at a roofing jobsite on a house at 266 Pennsylvania St., Buffalo, NY: Employees were working on the ground doing clean-up work in the vicinity of employees working above on the roof and a Ladder Jack Scaffold . The employees were exposed to the potential hazard of being struck by materials that could fall off the roof and scaffold. The employees were not wearing a hardhat. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) On or about 03/26/18, at a roofing jobsite on a house at 266 Pennsylvania St., Buffalo, NY: Employees were on the piks of a Ladder Jack Scaffold erected on the west side of the house. These employees walked on the pik to transport materials such as sheets of plywood to the employees doing the sheathing of the roof. The employees were exposed to a potential fall of approx. 18 ft. from the piks. No Fall protection system was being utilized. NO ABATEMENT DOCUMENTATION REQUIRED Titans Remodeling Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 11047041, citation number 1, item number 1a and was affirmed as a final order on 12/09/2015, with respect to a workplace located at 650 Columbus Parkway, Buffalo, NY 14213.
