29 CFR 1910.106(e)(2)(iv)(d): Flammable liquids were not drawn from or transferred into vessels, containers, or portable tanks within a building only through a closed piping system, from safety cans, by means of a device drawing through the top, or from a container or portable tanks by gravity through an approved self-closing valve:    (a) Near Wash Tanks - Employees dispensed acetone, a Category 2 flammable liquid, from a 55-gallon drum, onto cloth rags to clean letters off parts.  The 55-gallon drum of acetone had a nozzle that employees had to open and close rather than having a self-closing valve, exposing employees to a fire hazard, on or about November 10, 2015.    Corrected during the inspection.

29 CFR 1910.133(a)(2): The employer did not ensure that each affected employee used eye protection that provided side protection when there was a hazard from flying objects.  Detachable side protectors (e.g. clip-on or slide-on side shields) did not meet the pertinent requirements of this section:    (a) Paint Line - An employee who wore prescription safety glasses wore disposable, flexible plastic, slip-on side shields on the safety glasses.  These type of side shields do not comply with any American National Standard ANSI Z87.1 for impact protection, exposing the employee's eyes to a struck by hazard, on or about November 10, 2015.    Abatement certification required within 10 days after abatement date. 29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA:    (a) APX-York Sheet Metal Incorporated, 255 Church Road, York, Pennsylvania - An employee suffered lacerations and fractures to the face, resulting in hospitalization and surgery, on November 3, 2015 as a result of getting struck by an abrasive cut-off wheel that broke apart and flew off a portable pneumatic grinder.  The employer failed to report this work-related hospitalization to OSHA.    Corrected during the inspection.
