29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  (a) APX Fabrication Shop - The five emergency exit doors in the fabrication shop were locked using a fabricated door bolt made by the company, that required special knowledge to open, on or about February 6, 2020.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:  (a) APX Fabrication Shop - The emergency exit door in the fabrication shop near the employee entrance was obstructed by a grill that reduced the exit to 23 inches, on or about February 6, 2020.  (b) APX Fabrication Shop - The emergency exit door in the fabrication shop near the employee welding area was obstructed by a welding curtain that restricted the means of egress, on or about February 6, 2020.   No abatement certification or documentation required.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) Shop - Employees did set up activities, for example, die-setting on the Cincinnati 100-ton hydraulic press brake, serial number R51837 Model 60afx4ft, exposing employees to hazards, including but not limited to, getting caught-in and struck-by moving machine parts, and the employees did not utilize lockout/tagout when doing the set up activities, on or about February 6, 2020.  (b) Shop - Employees did set up activities, for example, die-setting on the Cincinnati 6ft 90-ton hydraulic press brake, serial number 51148, Model 90CBIIx6ft, exposing employees to hazards, including but not limited to, getting caught-in and struck-by moving machine parts, and the employees did not utilize lockout/tagout when doing the set up activities, on or about February 6, 2020.  (c) Shop - Employees did set up activities, for example, die-setting on the Accupress 10ft 130-ton hydraulic press brake, serial number 3068 Model 713010, exposing employees to hazards, including but not limited to, getting caught-in and struck-by moving machine parts, and the employees did not utilize lockout/tagout when doing the set up activities, on or about February 6, 2020.  (d) Shop - Employees did set up activities, for example, die-setting on the  Amada 14ft 130-ton hydraulic press brake, serial number N001110 Model HEF 130-45, exposing employees to hazards, including but not limited to, getting caught-in and struck-by moving machine parts, and the employees did not utilize lockout/tagout when doing the set up activities, on or about February 6, 2020.  (e) Shop - Employees did set up activities, for example, die-setting on the Amada 10ft 100-ton hydraulic press brake, serial number V000929 Model HFE 100-35, exposing employees to hazards, including but not limited to, getting caught-in and struck-by moving machine parts, and the employees did not utilize lockout/tagout when doing the set up activities, on or about February 6, 2020.  (f) Shop - Employees did set up activities, for example, die-setting on the Accupress 12ft 175-ton hydraulic press brake with lift assist, serial number 12660 Model 417512, exposing employees to hazards, including but not limited to, getting caught-in and struck-by moving machine parts, and the employees did not utilize lockout/tagout when doing the set up activities, on or about February 6, 2020.  (g) Shop - Employees did set up activities, for example, die-setting on the Amada 35-ton hydraulic press brake with lift assist, serial number 50356097 Model RG-35, exposing employees to hazards, including but not limited to, getting caught-in and struck-by moving machine parts, and the employees did not utilize lockout/tagout when doing the set up activities, on or about February 6, 2020.  APX - York Sheet Metal, Incorporated was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.147(c)(4)(i): which was contained in OSHA inspection number 1104773, citation number 1, item number 4 and was affirmed as a final order on May 4, 2016, with respect to a workplace located at 255 Church Road York, PA 17406.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.212(a)(3)(ii): Points of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zones during operating cycles:  (a) Fabrication Shop - Employees used a foot pedal to operate the Cincinnati 100-ton hydraulic press brake, serial number R51837 Model 60afx4ft, to bend work pieces, some of which required the employees to get to within 4 inches of the point of operation, and the press brake did not have point of operation guarding, exposing employees to the hazard of getting caught in the point of operation, on or about February 6, 2020.  (b) Fabrication Shop - Employees used a foot pedal to operate the Cincinnati 6ft 90-ton hydraulic press brake, serial number 51148, Model 90CBIIx6ft to bend work pieces, some of which required the employees to get to within 4 inches of the point of operation, and the press brake did not have point of operation guarding, exposing employees to the hazard of getting caught in the point of operation, on or about February 6, 2020.
