29 CFR 1926.451(g)(1)(iii): Each employee on a crawling board (chicken ladder) was not protected by a personal fall arrest system, a guardrail system (with minimum 200 pound toprail capacity), or by a three-fourth inch (1.9 cm) diameter grabline or equivalent handhold securely fastened beside each crawling board:    a)  Employees engaged in roofing work were exposed to fall hazards of approximately 13 feet to the ground while working from the crawling board.       In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

Style Exteriors, Inc. dba Style by Carden Exteriors, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 915951, citation number 2, item number 1 and was affirmed as a final order on August 21, 2013, with respect to a workplace located at 2858-2862 Meadow Ln., Schaumburg, IL 60193.    Style Exteriors, Inc. dba Style by Carden Exteriors, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 562138, citation number 1, item number 1a and was affirmed as a final order on September 26, 2012, with respect to a workplace located at 5013 W. Winnemac Ave., Chicago, IL 60656.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
