29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At the 7205 Hodgson Memorial Dr. jobsite: On November 12, 2015, the employer exposed an employee to fall hazards of 13 ft. 6 in. to the ground below, in that while accessing and egressing a flat roof from a 20 ft. extension ladder with side rails extended approximately 6 in. above the top-parapet wall, the employee was subjected to serious injury or death.
