On or about October 29, 2015, an employee was exposed to amputation hazards due to an unguarded set of drive belts. The employer did not establish a safety program that addressed on site equipment repairs and the safeguarding of power transmission components prior to the start up of equipment. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
