29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). On or about November 9, 2015, at an excavation site located between 4th and 5th Street and N. Main in Stillwater, OK, no protective devices were in place to protect employees while working in a trench, exposing employees to the hazard of being crushed by falling soil.
