Employees were exposed to an eye injury hazard while working with a Big Joe brand battery powered reach truck (model number PDR-20-130, serial number 382591) without access to an emergency eyewash station.

The outlet boxes lacked proper strain relief and were equipped with knockouts. The knockouts could be displaced by an employee's fingers during use.
