29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:   On or about November 13, 2015, during carpentry framing and soffit installation; the employer did not conduct frequent and regular inspections of the job site where employees were exposed to fall hazards and possible head injury from material falling from an elevated working surfaces with a height of approximately 13 feet.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   On or about November 13, 2015, at a job-site located at 2532 Ashwell Ave SW, Massillon, Ohio 44646; the employer did not ensure employees were protected from head injuries from falling loads by allowing them to work under the roofing contractors whom were installing roofing material while the companys employees also installed soffit material without the use of a protective helmet.
