On or about January 26, 2016, while performing framing activities on a newly constructed residential home located at 6502 Wisteria, Medina, Ohio 44256, the employer failed to protect employees from a fall of approximately 10 feet by requiring the use of a personal fall arrest system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about January 26, 2016, while performing framing activities on a newly constructed residential home located at 6502 Wisteria, Medina, Ohio 44256, the employer failed to protect employees from a fall of approximately 10 feet by extending the ladder used for access at least 36 inches over its landing area.
