29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: (a) Slitter Line 7 - Operators and assistant operators were not adequately trained as "Authorized" employees and/or knowledgeable in the recognition of set-up and minor servicing procedures that require machine or equipment shutdown and lockout to be performed to prevent injury from the unexpected release of hazardous energy. On or about November 9, 2015, an employee was injured while changing the mandrel on the Slitter Line 7 re-coiler. The employee's finger was inside the locking mechanism when another employee activated the control, which caused the equipment to move suddenly and pinch the employee's finger resulting in a finger tip amputation.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) Slitter Line 7 - On or about November 9, 2015, an employee was changing the Slitter Line 7 re-coiler machine set-up from a 20-inch mandrel to a 16-inch mandrel and did not: 1) prepare the machine or equipment for shut down, 2) turn off and shut down the machine or equipment, 3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source(s), 4) affix lockout or tagout devices to each energy isolating device, and 5) verify isolation and deenergization of the machine or equipment have been accomplished prior to starting service and/or maintenance work. Alternatively, the employer failed to develop and implement a procedure or procedures for removing the mandrel locking devices that provided the same level of protection as full lockout/tagout of the re-coiler. This practice exposed employees to caught-in and amputation hazards.
