29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems: On or about November 13, 2015, at the trenching site located at Sooner Road south of Danforth Road in Edmond, OK, a competent person was on site but did not conduct daily inspections of an excavation to ensure conditions did not exist that could result in possible cave-ins, exposing employees to the hazard of being crushed by falling soil.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about November 13, 2015, at the trenching site located at Sooner Road and Danforth Road in Edmond, OK, an employee was allowed to work in a trench more than 4 feet deep that had no ladder or other safe means access/egress within 25 feet of the employee. United Trenching was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.651(c)(2), which was contained in OSHA inspection number 567619, citation number 1, item number 1, and was affirmed as a final order on Februarys 6, 2013, with a final abatement date of January 15, 2013, with respect to a workplace hazard west of Bel Air on Hefner Road in Oklahoma City, OK 73120.

On or about November 13, 2015, at the trenching site located at Sooner Road and Danforth Road in Edmond, OK, no protective system was in place to protect an employee working in a trench that was more than 5 feet in depth, exposing employees to the hazard of being crushed by falling soil. United Trenching was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 567619, citation number 1, item number 2, and was affirmed as a final order on Februarys 6, 2013, with a final abatement date of January 15, 2013, with respect to a workplace hazard west of Bel Air on Hefner Road in Oklahoma City, OK 73120.
