On or about November 11, 2015 and at times prior thereto, at or near Spanish Rock Apartments, employees were engaged in roofing construction, exposed to a fall distance of greater than 6 feet to the ground below, without fall protection while removing shingles from a two story apartment building. This condition exposed employees to fall hazards.      Jose Perez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 232911, citation number 1, item number 2 and was affirmed as a final order on May 31, 2011, with respect to a workplace located at 2707 Oakwood Village Apartments, Mesquite, TX, 75150.

On or about November 11, 2015 and at times prior thereto, at or near Spanish Rock Apartments, 418 Markham Mesa Drive, Little Rock, Arkansas, employees used a metal extension ladder to ascend and descend from the second story roof that did not extend at least three feet above the upper landing surface.
