29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) Second floor, new townhome construction located at 2275 Tenor Lane, Alpharetta, GA 30009: Employees were exposed to eye injuries while installing a 3/4-inch OSB plywood subfloor with pneumatic framing nail guns without protective eyewear. Employees were exposed to struck-by hazards which included plastic collated debris and 2-3/8-inch fasteners as no safety glasses or goggles were provided.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Second floor, new townhome construction located at 2275 Tenor Lane, Alpharetta, GA 30009: Employees were exposed to fall hazards of approximately 22-feet, 7-inches to the lower level while installing a plywood subfloor without a fall protection system in place. Employees installing plywood near the edge were not protected from falls.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) Second floor, new townhome construction located at 2275 Tenor Lane, Alpharetta, GA 30009: Employees installing a plywood subfloor at heights of approximately 22-feet, 7-inches from the lower level were not provided with a fall protection system. The employees did not recognize the hazards and means to correct the hazard as the employees were not trained.
