29 CFR 1926.503(b)(1):     The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.     On November 16, 2015 employees engaged in commercial roofing operations were not protected from a fall, exposing employees to a fall of 12 feet.       Armor Shield Constriction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1005197, citation number 2, item number 1c, and was affirmed as a final order on April 16, 2015, with respect to a workplace located at 1600 E. Thacker St. Des Plaines, IL 60016.         Armor Shield Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard , which was contained in OSHA inspection number 314686064, citation number 1, item number 2 and was affirmed as a final order on August 25, 2010, with respect to a workplace at 77 Rand Rd., Des Plaines, IL 60016.
