29 CFR 1926.404(b)(1)(i):  The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:  a)  On September 7, 2022, employees were provided with electrical tools and equipment connected by cord and plug without the use of an assured equipment grounding conductor program or ground fault circuit interrupters (GFCI's). 29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  a)  On September 7, 2022, employees engaged in roofing activities were exposed to falls of approximately 20.7 feet.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Armor Shield Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.502(f)(1)(iii), which was contained in OSHA inspection number 1566090, citation number 1, item number 2a and was affirmed as a final order on May 3, 2022, with respect to a workplace located at 550 Lake St, Roselle, IL 60172.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
