29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:  At the Aurora, WI facility, the employer did not maintain an audiometric testing program by making audiometric testing available to employees whose noise exposure exceeded an 8-hour time-weighted average of 85 decibels:  a)  Chip & Spool employee, at bench #17, was exposed to noise at a TWA sound level of 91.2 dBA over a 147 minute sample time.  b)  Pedestal Grinder employee, at pedestal grinder #5, was exposed to noise at a TWA sound level of 88.2 dBA over a 143 minute sample time.  The Koller Industries DBA Aurora Casting Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.95(g)(1), which was contained in OSHA inspection number 278856, citation number 1, item number 1b and was affirmed as a final order on 06/05/2015, with respect to a workplace located at 1191 Country Road B, Aurora, WI 54151.
