(b)  On or about November 18, 2015, an exit route located at the north east corner of the building was obstructed with a part basket partially blocking the exit door.

29 CFR 1910.303(c)(3)(i): All splices and joints and the free ends of conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose:    (a)  On or about November 18, 2015 an industrial fan utilized at stand grinder two had a splice from flexible cable to metal clad wire that was partially covered with electrical tape.     (b)  On or about November 18, 2015 a portable basket tilting device utilized at stand grinder two had two splices made with wire nuts from flexible cable to metal clad wire that were partially covered with electrical tape.    (c)  On or about November 18, 2015, an abandoned energized metal clad wire above workstation 18 had exposed insulated conductors capped with wire nuts suspended in the air.  The conductors metal clad protection ended several feet from the free end and no other insulation and protection was provided.

The employees were exposed to struck-by hazards if the nut were to become lose and be projected off.  Employee(s) could also become caught in the rotating nut and wheel.     Aurora Casting Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.215(a)(2), which was contained in OSHA inspection number 278858 citation number 1, item number 3 and was affirmed as a final order on June 5, 2012, with respect to a workplace located at 1099 County Road B., Niagara, WI 54151.

(b)  On or about November 18, 2015 a portable basket tilting device utilized at stand grinder two was missing a faceplate on a junction box and the start button was pushed into the pendant allowing access to the unguarded electrical connections.      (c)  On or about November 18, 2015, numerous cord and plugged fluorescent light fixtures throughout the facility were not equipped with covers over the ballast and connections.
